Title of Project: Awareness Campaign Study

You are being invited to volunteer to participate in a research study. This summary explains information about this research:

• The purpose of this project is to evaluate the Family Advocacy Program’s Domestic Violence Awareness and Child Abuse Prevention awareness campaigns.

• If you decide to participate, you will be asked to complete online surveys four times over the next year. Each survey should take around 10-15 minutes to complete. These surveys will include questions to assess your knowledge, beliefs, and attitudes toward the campaign materials and content.

• Service members will need to complete the surveys outside of normal business hours (per OGC).

• You will receive a $10 electronic Amazon gift card via email after completion of the first survey, $15 after the second, $20 after the third, and $25 after completion of the fourth survey, for a possible total of $70 in gift cards.

• Survey responses will be stored in a secure database, and responses will be password-protected and only accessible to members of the study team. Any identifying information will be stored in a separate electronic location to reduce loss of confidentiality. Information collected in this project may be shared with other researchers, but we will not share any information that could identify you.

• The confidentiality of your electronic data created by you or by the researchers will be maintained as required by applicable law and to the degree permitted by the technology used. Absolute confidentiality cannot be guaranteed.

Your participation is voluntary, and you may decide to stop at any time. You do not have to answer any questions that you do not want to answer.
