The data collected helps health services researchers and policy makers understand changes in physician work environments, and related topics, such as participation in value -based payment programs, provider burden associated with documentation and EHR use, and the use of EHRs and how they affect the delivery of health care in the United States. The aggregated survey results shall be reported to Congress and help inform policymakers take steps to improve EHRs for patients and physicians.

  1. HOW WAS I CHOSEN FOR THE SURVEY?

You were randomly selected from a list of office-based physicians in the United States. This process ensures that the addresses selected represent physicians in the United States. Only sampled physicians or their proxy respondents can participate. Your participation is voluntary. However, we rely on you to make the survey results as accurate as possible.

  1. WHO WILL SEE MY ANSWERS?

We take your privacy very seriously. As required by federal law, your identity can be seen only by those NCHS employees and specially designated agents who need that information for a specific reason. No one else can see your answers until all information that could identify you and/or your practice has been removed. We combine your answers with other people’s answers in a way that protects everyone’s identity.

  1. HOW MAY THE DATA COLLECTED BE USED?

If you participate, you are giving consent for your deidentified information to be made available for future research by NCHS or other investigators. NEHRS is a survey on the use of electronic health records (EHRs) and the electronic exchange of health information among physicians in outpatient, and office-based practices in the United States. It is sponsored by the National Coordinator for Health Information Technology (ONC) and conducted by the National Center for Health Statistics, a Federal Statistical Agency, and a part of the Centers for Disease Control and Prevention (CDC). The data collected helps health services researchers and policy makers understand changes in physician work environments, and related topics, such as participation in value -based payment programs, provider burden associated with documentation and EHR use, and the use of EHRs and how they affect the delivery of health care in the United States. The aggregated survey results shall be reported to Congress and help inform policymakers take steps to improve EHRs for patients and physicians.

  1. WHAT LAWS PROTECT MY PRIVACY?

NCHS complies with numerous federal laws to protect your data, privacy, and confidentiality, as described in detail below. Data collection is authorized under Section 306 of the Public Health Service Act (42 U.S.C. 242k). A number of federal laws require that all information we collect be held in strict confidence: Section 308(d) of the Public Health Service Act (42 U.S.C. 242m(d)), the Confidential Information Protection and Statistical Efficiency Act of 2018 (44 U.S.C. 3561-3583), and the Privacy Act of 1974 (5 U.S.C. § 552a). Transmissions across the internet into Federal Information systems may be monitored for cybersecurity purposes in accordance with the Cybersecurity Enhancement Act of 2015 (6 U.S.C. §§ 151 and 151 note). Every NCHS employee, contractor, and agent has taken an oath to keep your information private. Anyone who willfully discloses ANY identifiable information could get a jail term of up to five years, a fine of up to $250,000, or both.


  1. WHO PROTECTS THE INTERESTS OF SURVEY PARTICIPANTS?


The Ethics Review Board (ERB) of the National Center for Health Statistics reviews survey content and methods to protect study participants. You may call the ERB to ask about your rights as a participant in this survey. The toll-free number is 1–800–223–8118. You will get an answering service. Please leave a brief message with your name and phone number. Say you are calling about Protocol #2023-01. Your call will be returned promptly.
