This information is being collected for the primary purpose of approving a Transportation
Service Provider (TSP) that is applying to be in the FEMA Standard Tender Service (FEMA
STOS) Program. The FEMA STOS Program allows FEMA to schedule and book transportation
with pre-approved private sector TSPs to move critical cargo and freight as needed to serve
FEMA’s mission to help people before, during, and after disasters.
The disclosure of information on this form is voluntary; however, failure to provide the
information requested may result in delay or denial of acceptance into the FEMA STOS
Program.
