On page 2 of our Application to Enroll and our Direct Reimbursement Request, we propose to clarify the Post Office Box instruction in the permanent residence street address field to match the cover page instructions for individuals experiencing homelessness whose Post Office Box may be considered a permanent residence street address. The change is non-substantive.

Track Change versions of both documents are included in this collection of information request along with clean versions of the revised documents. The following table crosswalks the change:
