Health and Injury Study
Public Reporting burden of this collection of information is estimated to be between 25 minutes for the web format and 40 minutes for the phone format per response, including the time for reviewing instructions, searching existing data/information sources, gathering and maintaining the data/information needed, and completing and reviewing the collection of information. Error Prompts.
Soft Prompts. Unless otherwise specified later in this document, if the respondent selects “Next>” without selecting an answer:

Required Questions. To be compliant with Section 508 of the Rehabilitation Act, all required questions will either followed by an asterisk (*) or by a verbal notice like this: “This question is required.”

What Sample Members See.

Emphasis Tags. To enable people using assistive readers to perceive intended emphasis, the screener and survey program will use the following html tags.
