Abstract: This is a request to update the name of the servicer, address, and phone number information from MOHELA to the U.S. Department of Education through out all six of the certification and suspension forms under OMB Control Number 1845-0158. There is no other change to the currently approved language of these forms. There is no anticipated change in the number of respondents/responses/burden hours currently approved for this collection.

Short Statement: ROCIS note: The program change comes to 53,013 responses and 17,494 burden due to removing an IC and adding it to another IC. The requested revision does not change the current number of respondents, responses, or burden hours. To reflect this statutory change, we have modified the Certification form by replacing all references to the highly qualified teacher requirement with the new requirement and removing the definition of “highly qualified teacher.” Minor conforming change in the TEACH Grant Service Obligation Suspension Request: Enrollment in a Qualifying Program or Completing Teacher Licensure Requirements removed a reference to the previously highly qualified teacher requirement, and a minor formatting change to the TEACH Grant Service Obligation Suspension/Discharge Request: Military Service by adding check boxes for the certifying military official to identify the specific type of qualifying military service performed by the TEACH Grant recipient.
