Paperwork Reduction Act Statement: This data is collected to assess the validity of your request for approval/
disapproval. It will take approximately 1 hour to complete this form. The collection is mandatory, and all
information collected shall be kept confidential. An agency may not collect, and a person is not required to respond
to an information collection, unless it displays a currently valid OMB Control Number. Comments concerning the

Instructions for FAA Form 1200-5
1. Enter the complete registered name of the business or organization associated with the request for
NAS data.
2. Enter full name of Point of Contact (POC) with delegated authority from requesting entity.
3. Enter POC’s full phone number and email address.
4. Enter alternate POC’s position, role, or duty title associated with the requesting entity.
5. Select either non-sensitive or sensitive NAS data. Reference definitions in FAA Orders 1200.22
and 1600.75. Note: For the purposes of this form, the term “sensitive NAS data” means NAS
data containing one or more types of Sensitive Unclassified Information, which includes aviation
security, homeland security, and protected critical infrastructure information. Non-sensitive NAS
data stipulates all data elements associated with sensitive NAS data are removed.
6. For Item 8a, indicate if this request is directly associated with a Federal agency or State, Local,
Tribal, or Territorial (SLTT) government entity, if applicable. If “Yes” for 8a, identify the specific
Federal or SLTT government entity in 8b.
7. Enter the requested start date to receive NAS data. Note: Depending on the type of NAS data
requested and purpose, processing timelines could range from 4 month to 24 months (estimated).
8. Requesters must obtain FAA sponsorship for any new request for NAS data, to include nonsensitive data not available from an existing external release system or sensitive data for limited
duration release for test purposes. Include POC name, phone number, and email for sponsor. Note:
Failure to obtain and identify required FAA sponsorship constitutes an incomplete request and
could delay processing.
9. The FAA seeks to establish your “need to know” for the type and category of data in your request.
Describe the type of organization or business associated with this request and the specific purpose
and intent that requires this requested NAS data. Indicate whether your request is for weather data,
radar data (specify facility or facilities), ADS-B data, etc. Attach any additional sheets required to
describe the purpose and rationale for the NAS data and include any Concept of Operations or any
other supporting documents which may help to justify the requestor’s “need to know.”
10. List full name and country of citizenship of non-U.S. associate and briefly describe their duties
associated with the NAS data request. Indicate “N/A” if not applicable.
