Facilitator Script: “Thank-you for participating in our Health Impact Assessment Training Sessions. We would like to gain further feedback regarding potential Environmental Justice Programs that Cuyahoga County Board of Health is considering in your area including the Health Urban Tree Canopy Program and Healthful Homes programs. Remember responses are voluntary and you can answer as little or as many questions as you would like.

Before we get started can each of you briefly fill out the notecard for the community you are representing, how long you have lived or worked in this community, your age range, and if you would like to be considered for participation in program activities.”

Focus Group Questions

  • General (5-10 minutes)

  1. What environmental burdens has your community faced and what is the most problematic at your home?

Potential questions for more clarity:

    1. Have these issues impacted you and your family’s health?

    2. What problems do you think are the most important to address?



  • Healthful Homes Program (5-10 minutes)

  1. What are some ways we can help make your home a healthier space?

Potential questions for more clarity:

    1. Do you have any concerns regarding your environment and health at home?

      1. Inside?

      2. Outside?

    2. Some examples may include improving air quality, decreasing risk of flooding, making homes more resistant to smoke from wildfires etc.
