Short Statement: OPM has consolidated several collections of information used to determine suitability for Federal employment, fitness to perform work for or on behalf of the Federal government, eligibility to occupy a national security sensitive position, eligibility for access to classified information, and/or eligibility for a Federal credential for access to Federal facilities or space and Federal logical systems. With this new common form, respondents will only fill out questions associated with their position risk and sensitivity. The common form replaces the forms known as the Standard Form 86, Questionnaire for National Security Positions [OMB Control Number3206-0005], the Standard Form 85P, Questionnaire for Public Trust Positions and the Standard Form 85P-S, Supplemental Questionnaire for Selected Positions [OMB Control Number 3206-0258], and the Standard Form 85, Questionnaire for Non-Sensitive Positions [OMB Control Number 3206-0261].
