LMD Community Feedback Survey Questionnaire

Intro Text	As we embark in a new year, the Office of Strategic Integration (OSI) would like to ask a few minutes of your time to help us focus our improvement efforts within the Logistics Management Division (LMD). Your voluntary action to complete this survey will help us better shape the support you receive from LMD Headquarters going forward. Completing this survey should take approximately 5-10 minutes and the survey will be open for responses from Wednesday 1/10/2024 to Thursday 1/25/2024. The survey is anonymous and all personnel, Civil Servants & Contractors performing logistics functions across all Centers, are invited to participate. Paperwork Reduction Act Statement: This information collection meets the requirements of 44 U.S.C 3507, as amended by section 2 of the Paperwork Reduction Act of 1995. The OMB control number for this information collection is 2700-0153 and it expires on 07/31/2024. Section 2 – Feedback This section explores specific LMD HQ’s activities. Start by selecting the LMD activity/function you interact with the most. You will be presented with 5 questions about that activity. Next, you will be brought back to the list to select another activity for which to answer the same 5 questions.
