Dear <First and Last Name>,

You have been selected to represent your fellow Sailors by participating in an important military research study investigating the health behaviors of male active duty service members. This survey is designed to be completed one time only. To show our appreciation for your time, you will receive a $40 gift card to Amazon.com for participating in the one-on-one phone interview. Your participation may also help us better address the needs of future Sailors.

This Department of Defense sponsored study will be conducted completely confidentially.

Any information that you provide will not be made available to your chain of command or promotion boards, nor will it affect any future care you receive.
