Dear Resident:
Your address has been randomly chosen to participate in the National Household Survey on
Behavioral Health (NHSBH)1 conducted by the U.S. Department of Health and Human Services (HHS).
Join the almost 70,000 people who are interviewed each year for this important study. NHSBH collects
data on mental health and the use or non-use of alcohol, tobacco, and other substances. Your
participation is voluntary and important for shaping the future of our nation's health services.
You can participate online or in-person, at your convenience from the comfort of your home. After
answering a few questions, zero, one or two household members may be selected for the interview.

Anyone who is selected and completes the full interview will receive $30.
To Participate Online:

Answer the initial questions to determine if zero, one or two members of your household aged 12
or older are selected to complete the full interview online. This will only take a few minutes.

4 Selected persons will be provided a unique link to begin that interview from any internetenabled device. Parental permission is required before anyone aged 12–17 can participate.

To Participate In-Person:
A professional interviewer from RTI2 may visit your address to provide more information. During this
visit, you can answer the introductory questions and if selected, complete the full interview.

Privacy
All answers you give are kept confidential by federal law and used for statistical purposes only. Your
responses are combined with thousands of other participants and reported as overall numbers.
