Abstract: VA form 29-1546 is used by the insured to apply for a loan and/or cash surrender. VA Form 29-1546e (DocuSign) will be used by the insured to apply for a cash surrender. The information requested on these forms is authorized by law, 38 USC 1906, 1944, 38 CFR 6.115, 6.116, 6.117, 6.100, 6.101 and 8.28.
