Dear recreational water colleagues,
We invite you to participate in an online survey about your jurisdiction’s public aquatic facility regulations
and experience using CDC’s Model Aquatic Health Code (MAHC). This survey is primarily aimed towards
epidemiologists and environmental health professionals working on public aquatic venue safety, regulation,
and inspection. Your jurisdiction’s feedback is important to us, even if you have limited or no direct
experience using the MAHC. If you have colleagues who may have relevant feedback on this subject, we
would appreciate you passing this survey on to them.
Participation in this survey is completely voluntary. This survey is estimated to take approximately 20–30
minutes to complete. If you need to save your responses and submit the survey at a later time, you will be
able to do so.
