Please take a few minutes to complete this web survey about your jurisdiction's regulations for public
aquatic facilities and experience using CDC’s Model Aquatic Health Code (MAHC). This survey is for
epidemiologists and environmental health professionals working on public aquatic venue safety, regulation,
and inspection, with any level of MAHC experience. Your responses will help CDC shape the MAHC into
a better resource for state, territorial, local, and tribal public health partners.
Participation in this survey is completely voluntary. This survey is estimated to take approximately 20–30
minutes to complete. Responses can be saved and submitted at a later time.
