Did you complete your Model Aquatic Health Code survey?
Reminder: Only 1 week remains to complete this voluntary survey about your jurisdiction’s public aquatic
facility regulations and experience using CDC’s Model Aquatic Health Code (MAHC). This survey will
take approximately 20–30 minutes to complete. If you need to save your responses and submit the survey
at a later time, you will be able to do so.
Your jurisdiction’s feedback is extremely important to us, even if you have limited to no direct experience
using the MAHC. If you have colleagues who may have relevant feedback on this subject, we would
