You were selected to participate in the Spina Bifida (SB) group discussion on <Date Time>. The purpose of the focus group is to learn about healthcare and other experiences among individuals with SB. The focus group will be held online using a tablet, laptop, or desktop computer with a camera. Some more information about the focus group is below.

• The focus groups will last approximately 90 minutes.

• We ask that you keep your camera on during the entire focus group.

• You will receive $75 for your time.

• The focus groups will be recorded only to help with note taking and will not be shared.

• Nothing said during the focus groups will be linked to your name.

• Your participation in the focus group is voluntary and you can refuse to answer any question for any reason. Also, you can stop participating at any time, for any reason, without penalty.

• The information collected will help the CDC to understand the healthcare experiences of people with SB, and shape projects to help people with SB get the care they need.
