Measures included here are cross-cutting tools. Once reviewed, they are mirrored in other tools such as Intensive.
Includes deeper measurements of the impact across short-term T/TA efforts and consolidated previous forms to create common measures and streamline reviews.
Two performance measures required through GPRA for the Agency captured across all SOAR offerings and will be administered to a larger population





2 Intensive T/TA Participant Feedback (Specialized T/TA) Sent to participants following a specialized T/TA, including HTLA, Recipient T/TA, SOAR for Organizations, SOAR for Communities, and other specialized T/TA. This tool combined pre-, post-, and mid-stream questions. Core Competencies, Guiding principles, Facilitator, Overall feedback, Satisfaction/overall quality/ expectations/usefulness/ preparedness, Plans, Barriers, Recommend OTIP/NHTTAC, Open ended questions

3 Follow Up Feedback   Combines and expands into detailed questions about outcomes 3-6-9-12 months following T/TA. This tool is cross-cutting T/TA types to be inclusive of both short-term and specialized T/TA. Replication of the Universal and Intensive T/TA forms More in-depth questions about application of material longer term, additional T/TA needs, and obstacles. Contains same items in the Universal and Intensive to assess applied skills and knowledge at 3-6-9-12 months following T/TA. New placeholders added to align with tailored competencies.


4 Qualitative Guide Consolidated list of questions for focus groups, interviews, and listening sessions. Contains questions about standard NHTTAC evaluation as well as special topics/projects. This allows for use in a variety of settings (including focus groups and listening sessions); potential questions about T/TA and places where questions can be adapted to fit new topics; special projects and areas for deeper exploration to develop new content areas (includes placeholders and items that can be adapted). 5 Network Survey Workgroup measures: Captures collaboration and coordination within a group of organizations or individuals on a common action area. For example, resulting connections and partnerships from SOAR for Organizations and Communities that occur following SOAR.

Peer measures: Captures developing partnerships and coordination efforts of an individual that were a result of T/TA. For example, resulting connections from HTLA/Recipients and collaboration among fellows/recipient organizations and their individual/agency networks.

7 Resources Feedback Universal tool to capture feedback on various resources and support provided by NHTTAC. This tool can be deployed after contact with the NHTTAC Customer Support Center, capture; experience with NHTTAC resources, reason they accessed the resource, and how they will use the information; and to understand website users’ experience with the NHTTAC website.
