Attachment C: Round 1 web survey instrument with parents

Welcome! Thank you for your interest in our research to improve the National Longitudinal Survey of Youth, a survey that follows American youth to collect information on labor market behavior and educational experiences.

The study should take about 10 minutes. Please only start the study when you will be able to complete the whole study without interruption. Please do your best to respond to the questions accurately. Depending on your responses, you may be invited to complete a follow-up survey.

Please do not use your browser's back button.

This voluntary study is being collected by the Bureau of Labor Statistics under OMB No. 1220-0141 (Expiration Date: July 31, 2027). Without this currently approved number, we could not conduct this survey. This survey will take approximately 10 minutes to complete. If you have any comments regarding this estimate or any other aspect of this study, send them to [email protected]. The BLS cannot guarantee the protection of survey responses and advises against the inclusion of sensitive personal information in any response. This survey is being administered by Qualtrics and resides on a server outside of the BLS Domain. Your participation is voluntary, and you have the right to stop at any time.

Next, we’d like you to imagine that your child was selected to participate in a survey for the federal government:

  • The survey will ask questions every few years about your child’s experiences at home, school, and work when they enter the labor force.

  • The information from the survey will provide valuable information about the educational, economic, and social progress of people from your child’s generation over time.

Before agreeing to allow your child to participate in the survey, you as the parent or guardian would sign a consent form. The consent form should clearly explain the survey so that you can make an informed decision about whether your child can participate or not.

What information would you expect to find on the consent form? (open-ended)

How would you expect to receive the information on the consent form? (randomize order except last option)

  • In-person with a federal employee

  • Over the phone with a federal employee

  • Over a video call with a federal employee

  • Signing a paper form

  • Another way – please specify
