These adjustments are due to a decrease in the number of intake forms submitted which resulted in a reduction to the total number of respondents by -3 respondents, the total annual responses by -3 responses, and the total annual burden hours by -9 hours. The total annual cost remained the same. There are no program changes to this information collection.
