NASA Safety Reporting System


If It's Not Safe, Say So!

The NASA Safety Reporting System (NSRS) is an anonymous, voluntary and responsive reporting channel to notify NASA’s upper management of your concerns about hazards. Your report is guaranteed to receive prompt attention.


Established by the NASA administrator in 1987 after the Challenger Shuttle mishap, NSRS has since supported all flights and has been expanded to cover all NASA operations.

The NASA Safety Reporting System (NSRS) is designed to supplement -- not replace -- your local hazard reporting channels. Please report any hazard or safety concern through your organization's established procedures before using the

NSRS. Local reporting often allows for quicker action, as Agency-level processing of a NSRS report may add additional time to respond.


Use NSRS Online Reporting if:

Shape2 Shape3 You have reported a hazard locally and have seen no action taken, You are not satisfied with the response to a reported hazard, or

Shape4 You fear reprisal if you were to report the hazard through local reporting channels.


Anyone may use the NSRS to report hazard or safety and health concerns affecting the NASA workforce, assets, or facilities, or the public visiting NASA locations by downloading and printing an NSRS Report Form from the NSRS website and submitting the form via the United States Postal Service. Only NASA civil servants or support contractors with access to the NASA network (onsite or by VPN) can submit hazard or safety and health concerns through this NSRS Online Reporting form.

This information collection meets the requirements of 44 U.S.C. § 3507, as amended by section 2 of the Paperwork Reduction Act of 1995. You do not need to answer these questions unless we display a valid Office of Management and Budget (OMB) control number. The OMB control number for this collection is 2700-XXXX; it expires on XX/XX/XXXX. We estimate that it will take about 15 minutes to read the instructions, gather the facts, and answer the questions. Send only comments relating to our time estimate to [email protected].


When you submit this form, it will not automatically collect your details like name and email address unless you provide it yourself.

Warnings and Limitations with NSRS eReporting

PLEASE NOTE: Complete anonymity cannot be guaranteed when submitting information over the internet.

Every NASA computer is subject to IT surveillance, and even with secure communications, there is always an auditable trail outside the NSRS team’s control. This includes details like IP addresses, Internet Service Provider (ISP) records, web proxies, and other records that may be stored by your ISP, network, or computer. If you prefer, you may submit a report by downloading and printing a physical NSRS Report Form from the NSRS website and mailing it with the United States Postal Service to the NSRS Post Office Box.


PLEASE NOTE: Do not include the following types of information in your report.

Shape6 FRAUD, WASTE, and ABUSE should be reported directly to the NASA Inspector General by dialing the Inspector General HOTLINE at 1-800-424-9183.

Shape7 The NSRS independent contractor will forward any report implying criminal activity to the NASA Inspector General.

Please describe the hazard or safety concern in detail. Include as much information as you can to help clarify your concern. Be sure to include the following information:

Provide the Center/Location of your hazard as well as the physical location including Building, Room, etc.

A description of the hazard or safety concern;

Whether the hazard affects NASA assets, the NASA workforce, and/or the general public; and

Whether or not you have reported the hazard elsewhere.

With many hazards or safety concerns, additional information might assist an investigation: Manufacturing sources and/or part numbers (if hardware is involved);

Whether the hazard relates to a single event or a recurring process; What you believe caused the hazard;

What you think can be done to correct the hazard and prevent a recurrence; or What you believe the consequences may be if the hazard remains unresolved.

    1. After receiving your report the NSRS contractor will provide you with a case number and a website link for status.

    2. The NSRS contractor may be requested to contact you if the information you provided about the hazard or safety concern needs clarification.


When it is determined that no further clarification about your report is needed, all identity information provided in your report will be permanently deleted to protect your anonymity. THE NSRS PROGRAM WILL NOT SHARE YOUR IDENTITY WITH NASA AT ANY POINT.
