NASA Safety Reporting System
If It's Not Safe, Say So!
The NASA Safety Reporting System (NSRS) is an anonymous, voluntary and responsive reporting channel to notify NASA’s upper management of your concerns about hazards. Your report is guaranteed to receive prompt attention.
Established by the NASA administrator in 1987 after the Challenger Shuttle mishap, NSRS has since supported all flights and has been expanded to cover all NASA operations.

The NASA Safety Reporting System (NSRS) is designed to supplement -- not replace -- your local hazard reporting channels. Please report any hazard or safety concern through your organization's established procedures before using the
Use NSRS Online Reporting if:
You
have
reported
a
hazard
locally
and
have
seen
no
action
taken,
You
are
not satisfied with the response
to a reported hazard, or
You
fear
reprisal
if
you
were
to
report
the
hazard
through
local
reporting
channels.
Anyone may use the NSRS to report hazard or safety and health concerns affecting the NASA workforce, assets, or facilities, or the public visiting NASA locations by downloading and printing an NSRS Report Form from the NSRS website and submitting the form via the United States Postal Service. Only NASA civil servants or support contractors with access to the NASA network (onsite or by VPN) can submit hazard or safety and health concerns through this NSRS Online Reporting form.
This information collection meets the requirements of 44 U.S.C. § 3507, as amended by section 2 of the Paperwork Reduction Act of 1995. You do not need to answer these questions unless we display a valid Office of Management and Budget (OMB) control number. The OMB control number for this collection is 2700-XXXX; it expires on XX/XX/XXXX. We estimate that it will take about 15 minutes to read the instructions, gather the facts, and answer the questions. Warnings and Limitations with NSRS eReporting
Every NASA computer is subject to IT surveillance, and even with secure communications, there is always an auditable trail outside the NSRS team’s control. This includes details like IP addresses, Internet Service Provider (ISP) records, web proxies, and other records that may be stored by your ISP, network, or computer. If you prefer, you may submit a report by downloading and printing a physical NSRS Report Form from the NSRS website and mailing it with the United States Postal Service to the NSRS Post Office Box.

Inspector
General

NSRS

contractor

forward
any
report
implying
criminal
activity
to
the
NASA
Inspector
General.
