General Instructions
The report must be submitted to the FMC within 30 days of the end of the month being reported.
The report template spreadsheet must not be modified in any way beyond the addition of the respondent's repo
For sailings that span months, the information should be filed in the month of the final U.S. port call.
Carriers who choose to report Canadian or Mexican port calls must provide data on all port calls in Mexico and/o
U.S. port call.
If a vessel is taken out of service for any reason, the data for that vessel’s port calls must be reported for the mo
service. In the spreadsheet, please input a row below the last port call before removal indicating “OOS” for colum
Data Entry Instructions
Vessel Name must be the vessel name exactly as it has been reported to Customs and Border Protection (CBP). D
Vessel IMO Number must be the seven digits after “IMO” for the reporting vessel.
Vessel Owner must be the vessel owner as reported to CBP.
Vessel Operator must be the vessel operator at the time that the port call was made as reported to CBP.
Gross metric tons are calculated inclusive of container weight.
All port call dates must be formatted using MM/DD/YYYY.
All reported data must be rounded to the nearest whole number, do not use decimal points or any non-numeric
Data Transmission Instructions
The monthly report must be saved with the file naming convention "Carrier Abbreviated Name_MM_YYYY.xlsx".
