Abstract: This request for a new information collection request covers multiple settings, including home health (HH), hospice, inpatient rehabilitation facility (IRF), long-term acute care hospital (LTCH), and skilled nursing facility (SNF) settings, Any Hospice, HH Agency (HHA), IRF, LTCH, or SNF - collectively referred to as providers - that does not meet the reporting requirements for their respective program may be subject to a payment reduction in its annual payment update. CMS provides HHAs, hospices, IRFs, LTCHs, and SNFs an opportunity to request an exception or extension from their respective program’s reporting requirements in the event they were unable to submit quality data due to extraordinary circumstances beyond their control. This request for a new information collection request includes burden for certain procedural requirements associated with the HH QRP, Hospice QRP (HQRP), IRF QRP, LTCH QRP, SNF QRP and the SNF VBP Program.
