Section 361 of the Public Health Service (PHS) Act (42 USC 264) (Attachment A1) authorizes the Secretary of Health and Human Services to make and enforce regulations necessary to prevent the introduction, transmission or spread of communicable diseases from foreign countries into the United States. Under its delegated authority, DGMH works to fulfill this responsibility through a variety of activities, including the operation of Port Health Stations at ports of entry and administration of foreign quarantine regulations; 42 Code of Federal Regulation part 71 (Attachment A2), specifically 42 CFR 71.20 Public health prevention measures to detect communicable disease.
On May 17, 2026, an outbreak of Ebola disease caused by Bundibugyo virus was detected in the Democratic Republic of the Congo (DRC) and Uganda. On May 20, 2026, the DHS published Arrival Restrictions Applicable to Flights Carrying Persons Who Have Recently Traveled From or Were Otherwise Present Within the Democratic Republic of the Congo (DRC), Uganda, or South Sudan. Airlines are instructed to redirect flights carrying persons who have recently traveled from or were otherwise present within DRC, Uganda, and South Sudan in the previous 21 days to Washington-Dulles International Airport (IAD). CDC is conducting public health entry screening at designated U.S. airports (IAD) of travelers coming from DRC, Uganda, and South Sudan. The purpose of public health entry screening is to detect ill travelers or travelers arriving from regions affected by the outbreak who are at risk of becoming ill with Ebola to facilitate post-arrival management.â¯
EVD is a severe illness in humans with extremely high case fatality rates. The scale of this EVD outbreak may continue to worsen. The best strategy to control the spread of the virus to the United States is to stop it at the source with core public health interventions, including case finding, contact tracing, and exit screening of travelers. As part of a layered intervention strategy, the U.S. government is supplementing activities in support of this public health response through the use of enhanced public health engagement and risk assessment for travelers arriving from countries affected by the EVD outbreak originating in the Democratic Republic of the Congo and Uganda.
To further protect U.S. communities from an imported case of EVD, the United States must be prepared to respond domestically by providing state and local health departments and healthcare facilities with the necessary assessments and questionnaires that can assist in identifying symptoms of or high-risk exposures to EVD in travelers arriving from areas affected by the outbreak. With assistance from the Department of Homeland Security, CDC will conduct these emergency assessments at designated U.S. airports that are receiving travelers from the outbreak areas. This system of risk assessment and triage will facilitate transmission of information to assist state and local health departments to complete a 21-day follow-up period and prevent introduction and spread of EVD into the United States.
