Instructions for FSA-211A
POWER OF ATTORNEY SIGNATURE CONTINUATION SHEET
Grantors use FSA-211A, Power of Attorney Signature Continuation Sheet, when the Grantor is an entity, such as a general partnership, joint operation, corporation, limited liability company, or other similar entity which require more than one member’s signature to appoint an attorney-in-fact (Grantee) to act on behalf of the entity.
Grantors must have their signature witnessed by an authorized Farm Production and Conservation (FPAC) employee, which consists of the Farm Service Agency (FSA), the Natural Resources Conservation Service (NRCS), and Risk Management Agency (RMA), or notarized by a Notary Public.
The completed original form must be attached to the associated FSA-211, Power of Attorney and submitted in hard copy to the appropriate USDA Service Center.
The original form must be submitted. FSA-211As received through telefacsimile machines or electronically will not be accepted.
The Grantor’s signature (Items 3A, 4A, 5A, 6A, and 7A) must be witnessed by an authorized FPAC employee or acknowledged by a Notary Public.

Name of Grantee
Enter the name of the attorney-in-fact (Grantee) from Item 1, FSA-211, Power of Attorney.
Item 2
Name of Grantor
Enter the name of the entity (Grantor) from Item 5, FSA-211, Power of Attorney.
Item 3A – 3C
Authorized Signature, Title, and Date
Each applicable Grantor must sign in Item 3A, enter their title/relationship in their representative capacity to the entity in Item 3B, and enter the signature date (MM-DD-YYYY) in Item 3C.

NOTE: Use the instructions above for Items 4A – 4C, 5A – 5C, 6A – 6C, and 7A – 7C, respectively.
Each Grantor’s signature must be witnessed by an FPAC employee or acknowledged by a Notary Public. 

FSA-211s and FSA-211As prepared and/or presented to NRCS for use in NRCS Conservation Easement Programs MUST be notarized. Participation in other NRCS programs does not require notarization.
Item 3D – 3F
Witness Signature, Signature Date, Official Position
For FPAC use only.

The FPAC employee shall sign in 3D when witnessing the signature of the Grantor. They should enter the date they signed in 3E and their official FPAC position in 3F.

NOTE: Use the instructions above for Items 4D – 4F, 5D – 5F, 6D – 6F, and 7D – 7F, respectively.

All FSA-211s and FSA-211As prepared and/or presented to NRCS for use for participation in NRCS Conservation Easement Programs, MUST be notarized. Participation in other NRCS programs does not require notarization.


Item 3G – 3I
Notary Public Signature, State, County, Commission Expiration Date, and Signature Date
For Notary Public use only.

The Notary Public shall sign in 3G when witnessing the signature of the Grantor; enter their State and county of commission; and enter the signature date (MM-DD-YYYY) in Item 3I.

The Notary Public’s commission expiration date must be entered in 3H unless their stamp or seal includes this information. 

The Notary Public must affix their stamp or seal to the FSA-211.

In general, a notary public’s certification must include:
    • acknowledgement ("acknowledged or subscribed before me")
    • State and county of commission
    • Signature
    • Date
    • the notary’s embossing seal or stamp
    • the notary’s commission expiration date.

Notes: Questions specific to State law requirements about notary publics should be directed to the Regional Attorney’s office or applicable Secretary of State’s office.
