OMB Control Number: 0583-0153 Title of Clearance: Public Health Information System Agency Form Number affected by Change Worksheet: FSIS Form 9060-5, Meat and Poultry Export Certificate, 9060-5B, Meat, Poultry, and Eggs Export Certificate, 9060-5A Export Continuation Sheet, 9060-5S Fish and Fish Products Export Certificate, 9060-5EP, Egg Products Export Certificate Summary of Non-substantive Changes: FSIS is requesting a non-substantive change to remove the expiration date from the listed forms due to industry concerns that it could cause shipments to be delayed or held. International transit can sometimes take two to three months. Exporters are concerned that if the expiration date printed on the form passes while a shipment is in transit, foreign authorities may interpret the form or certificate as no longer valid, even when PRA approval has already been renewed. This creates risk during collection renewal periods, when forms bearing the prior expiration date may still be in use with shipments in transit. This change would not affect the information collected or the estimated burden.
