Abstract: This form collects information from individuals requesting to embark on a U.S. Navy ship or submarine as a distinguished visitor or Friend of the Navy. The Navy uses the information to determine whether the individual is eligible and medically able to participate, coordinate the embark, contact an emergency point of contact if needed, and provide related Navy public affairs information.
