Dear Employer,
 
This is your Notice of Recordkeeping Requirements for the 2027 Survey of Occupational Injuries and Illnesses (SOII). You do not need to report any information now; just keep records. In January 2028, the Bureau of Labor Statistics (BLS) will ask you to report your establishment’s injury and illness information that we request that you keep during the 2027 calendar year.
 
What must you do now?
    • Keep a record of all Occupational Safety and Health Administration (OSHA) work-related injuries and illnesses (see https://www.osha.gov/recordkeeping/entryfaq.html) that occurred throughout calendar year 2027 at the establishment(s) identified in the attached PDF.
    • Use separate forms for each establishment. OSHA injury and illness recordkeeping forms are available at:  http://www.bls.gov/respondents/iif/forms/oshaforms.pdf.
    • Record optional race and/or ethnicity information that this survey will ask you to report that is not included on the OSHA forms.  
    • Use the records you will keep throughout the 2027 calendar year to complete the SOII when BLS contacts you again in January 2028.
