FOIA Certification of Identity and Consent Form

The collection is necessary under 5 U.S.C. Section 552a(b) to collect information from individuals requesting information under the Privacy Act (PA). The Department will use the information to provide documents that are responsive to a Privacy Act or FOIA/Privacy Act request under the Freedom of Information Act. This Information Collection Request (ICR) was previously approved under 1880-0545 and ED is requesting a new number since this ICR is now under the Office of the Secretary (OS). There are no changes to the currently approved form.
