The Census Bureau plans to use this form to comply with the Equal Employment Opportunities Commission's guidance to collect race and national origin data from applicants in order to evaluate possible barriers to equal employment opportunity for decennial census applicants. In addition, the Census Bureau plans to use this form to comply with Executive Order 13548, which is intended to increase Federal employment of individuals with disabilities.
