Abstract: FORM FNS-418, SUMMER DATA INFORMATION FOR THE SUMMER FOOD SERVICE PROGRAM FOR CHILDREN, PROVIDES INFORMATION ON WHCIH TO BASE A PROFILE OF SPONSOR TYPES, SITE TYPES AND MEAL TYPES IN THE SUMMER PROGRAM. AUTHORIZATION IS BASED ON P.L. 95-166. THE INFORMATION IS SUBMITTED BY EACH STATE OPERATING A PROGRAM AND IS USED BY FNS FOR ANALYSIS AND MANAGEMENT OF THE SFSP. FORM FNS-80, AGREEMENT BETWEEN SPONSORS AND USDA. ASSURES THE SPONSOR HAS KNOWLEDGE OF RESPONSIBILITIES THAT MUS
