TO RECORD CHARGES OF EMPLOYMENT DISCRIMINATION FILED BY CHARGING
PARTIES WITH THE COMMISSION. THE INFORMATION IS USED BY EEOC IN ITS
INVESTIGATON OF THE ALLEGATION RAISED IN THE CHARGE.
