Abstract: THE INFORMATION COLLECTED ON THIS FORM WILL BE AGGREGATED AND USED TO DETERMINE THE CHARACTERISTICS OF TVA PROGRAM PARTICIPANTS FOR BOTH INTERNAL AND EXTERNAL PURPOSES. IN-HOUSE ANALYSIS WILL AID IN PROGRES REPORTING AND DEVELOPMENT OF PROMOTIONAL STRATEGIES. EXTERNALLY, DOJ' TITLE VI (28 C.F.R. 42.406) REGULATIONS WILL BE FULFILLED WITH RACE/ ETHNIC, SEX, AND AGE DATA GATHERED.
