Abstract: THIS INFORMATION COLLECTION REQUIREMENT IS NEEDED IN ORDER FOR OSM TO HAVE SUFFICIENT KNOWLEDGE OF EXISTING OPERATORS TO DETERMINE THEIR ELEIGIBILITY FOR ASSISTANCE. THE INFORMATION IS USE BY OSM TO ASSESS THE OPERATOR'S WORKLOADS AND ABILITY TO DEVELOP COST EFFECTIVE CONTRACT AND WORK ORDERS DURING THE APPROVAL THE APPROVAL OR DENIAL PROCESS.
