APPROVAL MEANS THE PLAN MEETS THE REQUIREMENTS OF IRC SECTIONS 401(A) AND 501(A) AS WELL AS APPLICABLE PROVISIONS OF ERISA (PUBLIC LAW 93-406). THE INFORMATION REQUESTED IS NECESSARY TO DETERMINE IF THE PLAN SHOULD BE APPROVED.
