It does not appear to be appropriate to obtain all the information required on Form 3672 & 3672A for employee benefit trusts that use plans previously approved by IRS. For these, a short form identifying the approved plan should be sufficient. Please resubmit a request for such a form if it is feasible or explain why it is not. It may be necessary to obtain copies of approved plans for reference, if they are not already retained in IRS.

A DETERMINATION IS ALSO MADE ON WHETHER THE RELATED TRUST QUALIFIES FOR TAX EXEMPT STATUS UNDER SECTION 501(A) OF THE CODE.
