Abstract: THIS FORM IS AN ANNUAL INFORMATION RETURN FOR THE WINDFALL PROFIT TAX AND IS REQUIRED BY SECTION 150.4997-2 OF THE REGULATIONS. FIRST PURCHASERS (AND ELECTING OPERATORS) USE THIS FORM TO REPORT THE TAX INFORMATION FOR PRODUCERS WHEN THERE IS WITHHOLDING OF WINDFALL PROFIT TAX. PRODUCERS USE IT TO REPORT THEIR OWN PRODUCTION WHERE THERE IS NO WITHHOLDING. (IRC SECTION 4997.) DATA IS USED TO HELP VERIFY THAT THE REQUIRED INFORMATION HAS BEEN PROPERLY REPORTED.
