Abstract: AN APPLICANT WHO IS UNABLE TO SUBMIT FOR CANCELLATION ANY PREVIOUS PASSPORT STILL VALID OR POTENTIALLY VALID, MUST SUBMIT A SIGNED STATEMENT SETTING FORTH THE CIRCUMSTANCES SURROUNDING THE DISPOSITION OF THAT PASSPORT WHEN APPLYING FOR A NEW PASSPORT. BASED ON THE DATA RECEIVED ON DSP-64, PASSPORT PERSONNEL DETERMINE WHETHER A NEW PASSPORT SHOULD BE ISSUED.
