Abstract: THIS FORM IS USED WHEN APPLICATION IS MADE BY FUNERAL HOMES FOR THE FUNERAL EXPENSES OF A DECEASED DEPOSITOR. THIS FORM IS COMPLETED BY A RELATIVE OF THE DECEASED DEPOSITOR CERTIFYING THAT THE BILL SUMITTED BY THE FUNERAL HOME IS CORRECT. ENTITLEMENT TO THE FUNDS ARE BASED ON THIS DATA TO INSURE PROPER PAYMENT.
