Abstract: FORM IS NECESSARY TO DETERMINE EQUAL OPPORTUNITY COMPLIANCE STATUS OF PROPRIETARY INSTITUTIONS AND JOB-TRAINING ESTABLISHMENTS WHICH ARE APPROVED TO TRAIN VETERANS AND OTHER ELIGIBLE BENEFICIARIES, AND PRORIETARY INSTITUTIONS WHICH ARE RECEIVING FUNDS THROUGH THE DEPARTMENT OF EDUCATION. IT IS USED BY AGENCY COMPLIANCE PERSONNEL TO APPRAISE THE COMPLIANCE STATUS OF FACILITIES UNDER TITLE VI OF THE CIVIL RIGHTS ACT OF 1964 AND TITLE IX OF THE EDUCATION AMEND. OF 1972.
