Abstract: THIS INFORMATION WILL BE USED BY THE DISTRICT SUPERVISOR TO COMPILE THE WELL RECORD INFORMATION REQUIRED BY 30 CFR 250.38 AND 250.95, AND AS A REFERENCE TO EVALUATE REMEDIAL ACTION IN THE EVENT OF WELL EQUIPMENT FAILURE OR LOSS OF CONTROL OF THE WELL. THE GEOLOGIC INFORMATION AND WELL TEST DATA WILL BE USED FOR LEASE AND TRACT RESOURCE EVALUATION. RESPONDENTS ARE FEDERAL OIL AND GAS LESSEES.
