THIS DATA INCLUDES THE ALERT SYSTEM. THE ALERT FORM DD 1938 IS USED TO RECORD THE FAILURE INFORMATION. PRIVATE INDUSTRY AND GOVERNMENT AGENCIES SUBMIT AND USE DATA.
