Abstract: THIS FORM COLLECTS REQUIRED BACKGROUND DATA TO DETERMINE AN INDIVIDUAL ELIGIBILITY FOR ENLISTMENT IN ACCORDANCE WITH THE U.S. CODE, CFR AND COAST GUARD REGULATIONS. MEETS REQUIREMENT OF 45.01-34, TITLE 33, CFR AND COAST GUARD REGULATIONS. MEETS REQUIREMENT OF 45.01-34, TITLE 33, CFR AND WARNS APPLICANT AGAINST GIVING FALSE DATA WHICH WOULD MAKE THE ENLISTMENT FRAUDULENT.
