Abstract: THIS FORM COLLECTS REQUIRED BACKGROUND DATA TO DETERMINE AN INDIVIDUAL ELIGIBILITY FOR ENLISTMENT IN ACCORDANCE WITH THE U.S. CODE, CFR AND COAST GUARD REGULATIONS. MEETS REQUIREMENT OF 45.1, TITLE 33, CFR AND WARNS AN APPLICANT AGAINST GIVING FALSE DATA WHICH WOULD MAKE THEIR ENLISTMENT FRAUDULENT.
