Abstract: THIS FORM IS USED TO GATHER INFORMATION ON THE COST AND QUALITY OF FUE DELIVERED TO ELE CTRIC UTILITY PLANTS. THE RESPONSES ARE USED IN EVALUATION OF INDIVIDUAL UTILITY COSTS AND PRACTICES IN RATE CASES, AND IN THE REQUIRED PERIODIC REVIEWS TO INSURE EFFICIENT USE OF RESOURCES.
