ALSO, REQUIRED IS AN INITIAL RATE SCHEDULE WHEN ISSUANCE OF A CERTIFICATE IS REQUESTED, AND A NOTICE OF CANCELLATION OF RATE SCHEDULE WHEN ABANDONMENT AUTHORIZATION IS REQUESTED.
