Terms of Clearance: This information collection is approved through 8/31/87 only. Before it is resubmitted, FHWA must revise Form MCS-90 as proposed in paragraph 9-C of the current justification statement. Specifically, FHWA must delete the last sentence of the first full paragraph of the form ("The telephone number to call is: ________"). The space for the insurance company telephone number should instead appear directly belo the space for the name of the insurance company, near the upper left side of the form. In addition, as proposed in paragraph 9-C, the form must reflect any new definition of environmental restoration that FHWA may adopt.
