THE INFORMATION REQUESTED WI AID THE REGULATORY AUTHORITY IN THE DECISION PROCESS TO APPROVE OR DISAPPROVE A REQUEST TO DESIGNATE OR TERMINATE AND AREA. THIS INFORMATION WILL ALSO BE USED TO MAINTAIN THE DATA BASE AND INVENTORY SYSTEM.
