THESE RECORDS WILL BE USED BY EMPLOYERS, EMPLOYEES, PHYSICIANS AND THE GOVERNMENT TO ASSURE THAT WORK PLACE EXPOSURE TO ETO DOES NOT ADVERSEL AFFECT THE HEALTH OF EMPLOYEES.
