bottom of the second page of form MMS-4070. A copy of the revised
form must be submitted to OMB for our official files. This action
was taken to help ensure that the form need only be submitted to
MMS once, and therefore, imposes the minimum burden on the public
in accordance with 5 CFR 1320.
